Baptism Inquiry Checklist

1. Fill out and submit the following form.
2. Purchase and complete the Baptism Preparation Course (click here to access). Then, send a copy of the course completion certificate to Kelly Ott at kott@stmatthewtn.org.
3. Complete the Godparent Form (download here). Have your godparents' parish(es) complete the bottom part of the form. Lastly, either mail in or scan/take a picture of the form and email it to Kelly Ott.

Baptism Preparation Course

Parents who are looking to have their child baptized at Saint Matthew are required to take the Baptism Preparation Course. At least one parent must take the course. Click here to access the online course. Once you have completed the course, please send a copy of the Course Completion Certificate to Kelly Ott at kott@stmatthewtn.org.

Godparent Information

According to Catholic Tradition, godparents assist the parents in passing the Catholic faith on to the child being baptized. Godparents must be Confirmed Catholics in good standing with the Church. In addition, the godparents must:

1. Be at least 16-years-old and have received the Sacraments of Baptism, Holy Communion, and Confirmation (or have been received in Full Communion with the Roman Catholic Church);
2. Be a practicing Catholic in good standing with the Church;
3. If married, have a marriage that is recognized by the Roman Catholic Church;
4. Be someone other than the father or mother of the candidate who will model the Christian life and teach their godchild Church teachings.

There are typically two godparents (one man and one woman), but one godparent is acceptable. The godparents must be present at the baptism. A Christian witness may be present at the baptism but will not be recorded as the godparent.
